Clinical Practice Experience (CPE) Project, Part 2 – Older Adult Holistic Plan of Care (begin week three, due week four):

After completing part 1 of the CPE project, utilize the information to develop an academic paper for a comprehensive holistic plan of care for the selected older adult. The holistic plan of care will also include goals, holistic self-care interventions, challenges and opportunities for the older adult client, evaluation of the goals, and overall impact of the plan of care to improve outcomes.

Below is an outline to guide the paper:

Title page

Introduction (includes the purpose of the paper/what the reader should expect in the body of the paper)

Comprehensive Holistic Assessment (includes physical, psychological, environmental, and cultural/ethical assessment data)

Analysis of Assessment Findings (includes notable age-related changes)

Goals (includes at least one short- and one long-term goal)

Holistic Self-Care Interventions (clearly identifies at least four)

Challenges and Opportunities (includes coaching strategies to assist the client)

Evaluation (includes measures to monitor goals & impact of care plan to improve outcomes)

Conclusion (summarizes the paper without adding new information -includes the overall impact of the plan of care)

References (includes a minimum of five scholarly resources)

Submit an academic paper, with 8-10 pages of content (not including title & reference pages), using proper APA style. Refer to the grading rubric criteria.
