This assignment addresses the following SON End of Program Student Learning Outcome (EPSLO):

#5 Patient Safety: Generate patient safety plans based on patient safety goals.

In order to meet this outcome, students must receive 80% accuracy or 160 points.

Assignment Description:

This assignment is divided into 4 parts. When an individual is diagnosed with a chronic condition, they often receive help from informal caregivers (family members). These types of situations can be overwhelming for both the patient and the caregiver. During such times, patients and caregivers face many challenges, including a lack of condition-specific education and inadequate knowledge of resources. During these types of circumstances, patients can ‘fall through the cracks,’ which can lead to safety concerns.

Nurses can be the glue that connects patients and family caregivers with the resources and knowledge they need. This assignment provides students with the opportunity to assist a caregiver and patient experiencing a newly diagnosed, life-changing, chronic condition. Although you are not doing this in a work setting, you can still find important ways to use your knowledge of connected care to help someone. You will use what you have learned in this course to plan and help the caregiver and patient get the resources they need to be safe. Please keep in mind that this assignment is focused on the caregiver and how you can help them keep their loved ones safe.

This paper will be a compilation of the information learned over the previous weeks.  The application of knowledge acquired will be applied as you conduct the interview, educate and provide resources and synthesize the information in the final paper. The previously gathered sources will be cited in this paper

Content to be covered:

Describe the caregiver’s and patient’s scenario -refer to them as ‘the caregiver’ and ‘the family member’. Describe briefly the interview: such as where it took place, who did you talk with, how long the interview was, and what your overall conclusions were. If you are uncomfortable with disclosing the interviewees first initial and last name in citation, you may use a psuedonym.

Briefly identify and summarize the chronic condition that is affecting the family member. Please give a short overview of the chronic health condition: what it is, how it is treated, and the expected outcomes. Also, identify any potential safety issues typical to this type of patient. Appropriately cite this information

Describe what a caregiver is. Describe your caregiver’s needs and/or burdens and/or stresses. Describe the coping methods this caregiver uses. Appropriately cite this information.

Advocacy: Provide information about advocacy and what it means to be an advocate. Use examples from your interview with the caregiver that demonstrates how that person advocates for the family member. Appropriately cite this information.

Describe typical, expected, or usual resources for caregivers to help themselves.  Describe what resources the caregiver is currently utilizing and how they are helping.  What two (2) new resources can you refer the caregiver to?  How did they respond to your suggestions? You may also describe any resources that could help the caregiver provide safe care to the family member. These resources need to pertain directly to the situation. Provide specific information about the identified resources, such as; what services do they provide? Where are they located?  Tell if there is any cost associated with using these resources?  Are they accessible to the caregiver? The resources can be local and/or national. Appropriately cite this information.

Discuss how care coordination principles can be used to help patients and caregivers in the community. How can care coordination improve patient safety in the community setting? Appropriately cite this information.

Describe how YOU, as the registered nurse, can advocate for the caregiver and the patient in this scenario. Information needs to be specific to the situation. (This section can be written in the first person) 

Requirements for Paper Format:

Write a 5 to 6-page APA formatted paper. Limited page numbers encourage the student to be organized and concise in their writings. The total number of pages does not include the title and reference pages.

The paper formatting must include a correctly formatted title page, headings, an introductory paragraph, the body of the paper, a conclusion paragraph, and a reference page. An abstract is not required. Introductory paragraph- State the purpose of the paper. What is the main reason for writing the paper? Explain the major points briefly covered in the paper and why readers should be interested in the topic. Body of paper- Present the required information. Every point should be supported with examples from your caregiver &/or patient. Cite the information and include the cited references on the reference list. Use APA formatted headings that reflect the content areas listed above. Conclusion paragraph – Summarize the key points in the body of the paper. Do not introduce new information in the conclusion. Headings- Follow correct APA formatting for all headings.

The paper should have a minimum of six (6) peer-reviewed journal articles. Required readings from the textbook should also be used but do NOT count towards the minimum of six peer-reviewed journal articles. You are welcome to use informational websites, but they will NOT count towards the minimum of six peer-reviewed journal articles. All references must be cited within the paper.

The paper should be written in third person, not first person. The information for section 7 listed above can be written in first person. Although the paper will be based on the interview, do not write the paper in a conversational format.

Do not cut and paste the information sources into this paper.  It is expected that pertinent information from the research will be paraphrased or summarized, correctly cited and will correlate with the information in the paper.

Use scholarly terminology in the paper. Avoid the use of colloquial expressions and slang. Please maintain professionalism and scholarly terminology at all times.
