Scenario: You are a nurse in charge of your organization’s palliative care program. You recognize that patient/family understanding of complex life limiting conditions, specifically with symptom management, and effective self-management strategies may be missing. Therefore, you are building an education program that will offer patients/family caregivers a better understanding of the problem. You will be hosting small group sessions where you will be providing your expert knowledge through a professional presentation method. This presentation is intended for patients and their family caregivers; therefore, the presentation needs to have factual, clear information to guide them. It is suggested to use the headings below to guide your presentation. Adding graphics and images will enhance your presentation – especially for patients and families. You may submit a voice over Power Point Presentation OR include talking points need on each slide’s notes page that covers additional details you want to convey to the audience.

Patient Role in and Strategies for Symptom Management

You will choose three to four symptom management challenges that are presented in Sherman & Matzo (2018) Physical Health: Symptom Management Chapters. The book chapters will provide you with knowledge about the symptom. You may need to use additional resources to supplement your knowledge. For each of the symptoms provide the patient/family caregiver an overview of the problem and what strategies may help self-manage the problem. Uses 4 or more references.

Resources and Prevention Strategies

Describe strategies that may be helpful to prevent medical, emotional, role, and financial strain. Discuss ancillary supports available. Summarize key points for patients, families, and caregivers. Uses 4 or more references.

 

Communication

Include ways to foster effective communication to prevent conflict and address incongruencies or confusion with the plan of care. (Family care conferences, reviewing end-of-life preferences, etc.). Uses 2 or more references.
