ICI Change Control Procedures

All new major designs go through a rigorous multi-disciplined review process which includes Hazard & Operability Studies (which ICI invented)

All other modifications and changes to the plant have to go through the minor modifications procedure and be approved an authorised by a multi-disciplined team of people. The procedure includes a risk assessment. Minor risk modifications can be fast-tracked.

A modification is defined as not replacing like with like. So replacing a broken Rosemount transmitter with an identical one would not require a modification but replacing it with a Foxboro one would. Modifications can be to hardware, software, management procedures or standard operating procedures. They can be permanent or temporary.
