Include the following: Name of the instrument or type of test 
Purpose of the assessment 
Critical evaluation of the assessment Was it reliable? 
Was it valid? 
If you received the results of the test, was the outcome (in your opinion) accurate? If not, what was not accurate? 

Avoid using assessments that take place online unless you can justify the formal nature of the assessment.
