Section #1: Screening, Intake, Orientation, and Record Keeping.
1-2 Paragraphs; briefly outline the more salient issues taking place. Use ASAM’s multidimensional assessment, all six areas (as applicable).
Section #2 Assessment and Treatment Planning
3-4 Paragraphs; identify any tools used and outcomes, as well as any criterion met from the DSM 5-TR diagnosis(es). Also identify then how this information will formulate a meaningful treatment plan (two problem areas, two goals for each problem area, two objectives to meet each goal) that follows theory and logic.
Section #3 Counseling and Client Education
2-4 Paragraphs explaining the intended format as well as theoretical orientations of counseling techniques, and expected client outcomes (i.e., what is the prognosis with this client?). Identify Educational programs and information that may be pertinent to the client.
Section #4 Case Management, Crisis support and Consultation & Referrals
2-3 Paragraphs; Identify what case management services are needed. Identify what plans are developed for Crisis intervention support. And recognize potential referrals either to supplement current treatment protocols or for aftercare services.
