OVERVIEW OF THE ASSIGNMENT:

This assignment will be presented as a digital poster in-person on a selected Wednesday on campus (an invitation will be sent to cohorts and faculty to attend) and will count towards your community clinical hours. Each group will be given 20‐30 minutes to present their electronic poster. There is a grading rubric attached to this assignment be sure to review where the majority of the points are reflected when working through this assignment (see grading rubric). The electronic poster must contain the information and have headings clearly outlining each of the various sections below (meaning all components need to be addressed to receive full credit for the assignment). The idea is that you are choosing an at-risk/vulnerable population who experience health disparities in the Hialeah/Miami area (you cannot use the same population or community as you did for your community assessment). There are a variety of populations you may choose from including elderly, teens, urban communities, homeless, migrants, immigrants etc (you should chose a population that is of interest to you or one that you might have personal knowledge about or have observed in your clinical setting). THE SELECTED AT-RISK POPULATION MUST BE APPROVED BY INSTRUCTOR PRIOR TO BEGINNING PROJECT (make sure to have rationale and support for how this population is considered at risk in Hialeah/Miami area). (See additional resources for health disparities websites at the end of this document).

Your group is responsible for reviewing the literature and researching further to better understand the needs and challenges of the chosen population. Using this information and current nursing evidence your group will develop a health care project/intervention, often a health-related teaching activity. Then you will evaluate the effectiveness of this proposed intervention. This process along with the proposed project/intervention will be outlined in the electronic poster and presented on campus by the group (each group member must be present). Your grade will include both the electronic poster submission along with resources to be used for the planned intervention as well as the presentation itself.

Personal reflection (individual): Each group member will submit a 200–350-word reflection describing how this project has changed influenced his/her view of health disparities and vulnerable populations in the local community and the role of the community health nurse.

SECTIONS THAT NEED TO BE ADDRESSED IN THE ELECTRONIC POSTER:

INTRODUCTION: This section involves identifying a vulnerable population that is found in the Hialeah/Miami area along with the major health problems/disease conditions amongst this vulnerable population and presenting relevant statistics that demonstrate both the health problems and health disparities that exist within this population.

Identify a specific vulnerable population that is found in the Hialeah/Miami area (this cannot be the same community you did for your community assessment project).

Provide a general overview of this vulnerable population along with your rationale for choosing this particular population (create a picture of what this population looks like here in Hialeah/Miami).

Provide an overview of the top health care problems and disease conditions found amongst this specific vulnerable population and briefly support/explain why these specific conditions affect this population supporting these findings with statistics and nursing evidence.

Identify a priority health problem, give a brief overview of the specific health problem that your group will address through a proposed project/intervention. (make sure to support with statistics and data why this is considered a priority health problem for this particular population in Hialeah/Miami)

Use a health risk or other health data to identify and prioritize one health problem within your vulnerable population that your interventions will address. State the health problem as a community health diagnosis using the four-part community diagnosis format (again you have done this before in Part 2 and 3 of your Community Assessment). This should be prioritized based on the identified health disparities within the vulnerable population and be well supported by your literature search, social determinants of health and the Healthy People 2030 goals.

Write the purpose of this poster presentation for your group

SCOPE & SIGNIFICANCE OF THE PROBLEM: Describe the impact on health that the health disparity as well as the identified health problem has on this vulnerable population which includes the relevant social, environmental, economic, and cultural issues.

Describe the identified health problem in detail (make sure to include relevant data and statistics and information from your review of the literature and research)

Identify and provide an overview of the major health disparities and factors that have led to health disparities among this population from within the context of the group’s identified health problem.

(Meaning what healthcare disparities further exacerbate and make this a larger health problem for this specific vulnerable population. Articulate and analyze direct and indirect factors that affect the context, disparity, and vulnerable population (this discussion should include the role of the social determinants of health in relation to health disparity and vulnerable population).

Considerations of the following should be included in this section: biophysical, psychological, physical environment, sociocultural, behavioral, health system

Significance:

Why is this health problem and vulnerable population of significance to the community

Why is this health problem within this population of significance to the nursing profession/health system? Discuss the relevance of this information to population‐centered healthcare providers (how does this information inform nursing and healthcare practices)

FINDINGS IN THE LITERATURE (may want to present this as a diagram or chart in the poster to save space and condense information)

Identify current interventions, education, and evaluation programs that are targeted to the health problem, disparity, and the chosen vulnerable population. Provide a brief overview of the relative strengths and weaknesses of these programs. (This discussion may integrate Health People 2030).

THE PROPOSED PROJECT: Develop a proposed project that your group plans to do to address this health problem in light of your understanding of the needs and healthcare disparities facing this vulnerable population within Hialeah/Miami area, the state of Florida and from the perspective of a community health nurse. This proposed project/intervention can be complementary to an already existing intervention, but you want it to be novel in its integration of cultural competency and bold use of the unique role of the community health nurse in reaching its goals.

Define the specific population you chose (if applicable this may already be made clear in your introduction)

Support the need for this type of project/intervention (based on your assessment findings from the literature review, statistics about this vulnerable population and the health problem and identified health disparities)

List specific goals and objectives for the project/intervention you seek to achieve in addressing the identified health problem/community diagnosis making sure the objectives are written in the SMART format (Specific, Measurable, Achievable, Relevant, and Time-bound; refer to Box 9.5 in textbook) which includes recognizing the needs of the vulnerable population and knowledge about the health disparity)

Detailed description of the proposed project/intervention to address the health problem in light of the identified health disparities for this vulnerable population and this must be in the form of a health related teaching activity involving providing education in various ways either through conducting a health screening or hands-on teaching at a local organization that you would identify meaning this cannot simply involve handing out brochures though that can be part of it this needs to involve you using your nursing knowledge to educate members of the vulnerable population with what you have learned about the population, the health problem and health disparities within this at-risk population (please see detailed description of what this should look like below)

Briefly explain how your project/intervention would integrate culturally competent nursing care

Explain how you plan to implement the project

CONCLUSION:

The impact the project will have on the identified population

Project Evaluation Methods: State the project measurements/outcomes: State the outcomes you expect to achieve based on your SMART objectives. State how you measure/assess the effectiveness of the project. (Again, this is similar to what you did as part of your Part 3 Community Assessment). What specific data/tools/strategies did or would you use to evaluate the project/intervention effectiveness (how would you know if your intervention/project was successful or not?)

Identify some potential challenges or barriers your group as community health nurses might have in attempting to carry out this nursing intervention/education. Consider how some cultural factors or other factors specific to your vulnerable population may interfere with adaptation of a new health practice and ways that the cultural health beliefs and practices may not fit with standard western biomedical care practices or standard health care facilities.

ADDITIONAL DETAILS AND GUIDANCE: (please take a few minutes to read through this)

As a professional electronic poster (as if you were presenting this at a nursing conference because you will be presenting to an audience) with the main elements outlined in each section presented and clearly outlined within the poster (see examples of general poster formats and I included a link for blank posters, so you have the ability to create your own). Remember the poster needs to include all the required elements. The poster must be done so that only major points are outlined (not too wordy or busy) most of this information will be presented using bulleted items within the poster; is visually appealing and has headers that address each specific part outlined below). I have provided links to help you understand the best way to design a poster (see link in Canvas). The poster must contain a reference list in APA (7th ed.) format. This can be within the poster itself or if there is not enough room can be included as a separate word document, but these references need to be submitted with the final project. The poster should have Florida National University and have all your group members names included (see examples). I also HIGHLY SUGGEST reviewing how posters are supposed to look you do want this to look professional!!!!!

Remember that you are representing the senior class and your cohort. Recognize that you will only have so much room on the digital poster and the poster is meant to guide and generally present the information and proposed project; but it will be your presentation that fills in the specific details. Meaning that when designing the digital poster recognize that it CANNOT be too busy; and it will NOT be able to contain every little piece of information as if you were writing a paper. You need to include bullets and the main points in the digital poster just enough to address the key points of your work BUT the detailed information will be filled in when you present the digital poster. Enough information needs to be included so that the audience can recognize and follow your group’s verbal presentation. This assignment will require you to put a lot of effort, you are representing your groups to the deans, other cohorts, other instructors and you are representing me as my class, so I have high expectations of your efforts and final projects. I expect you to treat this assignment and devote adequate time, effort, thought and mindfulness, taking pride in your work.

PROPOSED PROJECT:

Describe in detail the plan, strategies, methods, and specific interventions that your group plans to use in addressing this health problem and health disparity within the identified vulnerable population. You will be presenting this intervention in person on campus as part of your community hours. Imagine that you will be presenting this intervention to a local health organization that works with the identified at-risk/vulnerable population, so it is very important to have everything logistically described and specifically designed to address the identified health problem in light of what you have learned about your vulnerable population including the health problem and healthcare disparities your group has identified within this specific population. Explain the rationale for selecting these methods and interventions which are culturally focused.

Now you may develop a targeted community health intervention that may be complementary to those already existing but IT MUS BE novel in its integration of cultural competency and bold use of the unique role of community health nurse in reaching its goals and whichever health intervention you are modeling NEEDS TO BE ACKNOWLEDGED SO THAT YOU ARE NOT CLAIMING THIS INTERVENTION AS YOUR SPECIFIC IDEA UNLESS IT IS (meaning that if you identify an intervention from your review of other programs that you feel would work for your vulnerable population than you must adopt it accordingly and then give credit for where the idea for your planned project originated)

It is important to be very specific about the interventions your group are planning to provide. This is not a generic outline of an intervention BUT RATHER A SPECIFIC OUTLINE that includes a detailed discussion of specific interventions aimed at achieving your identified SMART objectives all of which are congruent and realistic in relation to the specific health problem, population, culture and health disparity (this includes teaching materials, location, time, translators if applicable, and other factors as if you were going into the community to carry out this intervention; recognize this is very similar to what you did as part of your Part 3 Community Assessment Assignment and your group will have my feedback from that assignment to help guide you for this assignment).

EXAMPLES OF PROJECT/INTERVENTION IDEAS:

For example, as part of your educational plan your group may decide to create an educational pamphlet/brochure to be distributed in a specific community which your group will identify and support with evidence; however, the choice of using a brochure needs to be specifically detailed and the brochure itself needs to be created by the group or if there is an organization that has existing resources you can use these brochures or pamphlets. Either way, all resources and your VERY SPECIFIC AND DETAILED PLAN to carry out the educational activity/project MUST BE SUBMITTED VIA A WORD DOCUMENT WITH YOUR FINAL PROJECT!!!!!

The use of any written form of intervention for your populations needs to not only integrate cultural components but also health literacy within the identified population. Perhaps the planned intervention involves attending a well-attended cultural event and your group will provide health screenings at this event. Your group will need to identify which health screenings you would be performing with rationale for why this screening is important; how education to the vulnerable group will be provided during this health screening; how follow-up will take place if during the screening something abnormal is identified. An outline of each group members responsibilities needs to be outlined. How will you obtain the supplies and equipment to carry out the health screenings? (would you partner with a local group? Hospital? Community organization? Which ones? What day of the week would the screening occur; how many people do you anticipate attending the event? How many hours will your group be available for the screenings?). DETAILS, DETAILS, DETAILS, (again the general overview of this intervention is presented in the poster, the details will be discussed during the presentation and provided to me in a word document submitted with the overall assignment). If written information like pamphlets or brochures will be provided perhaps an organization has them available for your population free and you can access these but these resources need to be uploaded with the assignment and should be provided as examples at the presentation so the audience can see what you would be presenting and using to educate the vulnerable population about the priority health problem). REMEMBER you group must be cognizant of the health disparities amongst this vulnerable group; for instance, if the group is not likely to have transportation than planning a health screening at a location that is in another county is not helping address the health problem or health disparity within this vulnerable population).
