I need  a 150 word reply to this  When a patient is unable to make their own decisions, a designated power of attorney can make the decisions for them. Always keeping the patient’s best interest in mind, the power of attorney can refuse any treatment or continue to consent doctors from doing everything possible to keep the patient alive. According to Jesson Rains Law (2015), there are some people who are authorizes to consent to medical treatment on behalf of an incapacitated patient. Some examples of people who are authorized are a court-appointed guardian, the patient’s spouse, a patient’s attending physician, and a patient’s reasonably available parents who are at least 18 years of age (Jesson Rains Law, 2015).            In the presented case, the patient’s husband is the one who can make the decisions of care for his wife. Although her daughter wants to keep her mother on a ventilator, the spouse is the one who can essentially make the final decision. He is the one who knows what the wife would want, especially if she has advanced directives for her husband to do as she wants. Furthermore, the ANA Center for Ethics and Human Rights (2016) states that patients usually have a conversation with their spouses about how they want their End-of-Life care to be. It is understandable that her daughter wants to keep her alive since the patient is her mother, however, if the patient had already told her husband what to do in case this occasion happened, then her daughter should respect her decision.
            The right thing to do, in my opinion, is to have a family intervention between the spouse and the daughter to come to an agreement about the patient’s treatment. Sometimes family members make decisions based on emotions and not from what the patient would really want. Being able to communicate and understand that perhaps the decision that the spouse is making is because that is what the wife would have wanted. Hopefully, by doing this intervention they can agree on what is in the patient’s best interest.

End of life care is difficult when it comes to families and their hopes for their family member. Like the example given, the grandfather would like to take his wife off the ventilator so that she is able to receive comfort care. “It is care that helps or soothes a person who is dying. The goals are to prevent or relieve suffering as much as possible and to improve quality of life while respecting the dying person’s wishes” (Department of Health and Human Resources, n.d., pp.2). In this case, the patient is diagnosed with respiratory failure, which means, “respiratory failure is the inability of the respiratory system to adequately supply fresh oxygen or remove carbon dioxide, resulting in low blood oxygen or high blood carbon dioxide levels, respectively” (Healthgrades, 2021, pp.2). Therefore, the patient is likely to not recover due to age and diagnosis. As a future nurse, I would assess the patient and discuss with the patient about their wishes for the future. By educating the patient that the condition may not change, this may help the patient decide what she would like to do. If the patient is unable to make choices for herself then it falls on her POA (Power of Attorney), which in this case, is unclear. Although the patient’s granddaughter made threats to sue the hospital, it may not be her choice what happens. If the patient’s husband is her POA and makes the choice to put her on comfort care, then the family should discuss and learn this may be the best choice for their grandmother. Choices like this can be difficult, therefore, nurses should give as much education as possible when appropriate.
