Here you want to identify the problem or health risk faced by your community.

For example: If your community health project is focused on proper hand hygiene to combat the spread of COVID-19, then “Risk for infection” would be a good community diagnosis to correlate with your project.

Here you want to identify the specific community client with whom you will be working in relation to the identified and stated problem.

For example: If you are teaching 1st grade students at a local elementary school, “adolescents of Lake County” would be a good example of the community population you are trying to aid in.

Here you will describe the characteristics of the community identified during the assessment phase. These may include motivation, knowledge, and skills of the community and its environment. Environmental characteristics include physical, cultural, psychosocial, and political characteristics.

Some of the identified characteristics may be strengths that you will build on to address the problem, while others contribute to the identified problem

For example:

Some age-related items you might find:

If working with first graders they are working at a disadvantage of an age-related knowledge deficit.

If working with high school students are known to partake in risk taking behaviors.

This is a good section to display the data and supporting items you have found during your community assessment.

For example:

Strengths:

Community has great access to healthcare with a multiple hospitals, clinics, and providers to assist patients with their health-related needs.

Weakness:

Heart disease is number one cause of death in community.

Fast-food restaurants noted in all areas of city.

Strengths:

-Availability of programs to encourage wellness check and educate residents about how to care for themselves. And learning how to use precautions to reduce rate of infection.

Weakness:

-Low rate of resident of the community attending free wellness check-up. Which bring about the High rate of resident visiting hospital for infection and other illness that could have been prevented.

Diagnosis:

Here you will combine the information from “risk of”, “among“, “related to” and “evidence by” to form a nursing diagnosis. Make sure that the diagnosis addresses the problem at the aggregate level – this distinguishes it from nursing diagnosis inspired by individual patients.

For example:

Risk of infection, among adolescents living in Willard, MO, related to knowledge deficit on communicable disease transmission, as evidence by improper hand hygiene techniques and increase in COVID-19 positive cases in elementary schools.

Risk of Infection among senior residents living in the community related to knowledge deficit due to high rate of uneducated resident evidence by high rate of hospitalization for infection and lack of resource usage.

Objectives or Goals:

Here you will discuss the objectives that your community project will address.

You need to develop a minimum of 2 short term goals and 1 long term goal.

These need to be in a SMART goal format.

This means they need to be specific, measurable, achievable, relevant, and time-bound.

Short term goal example:

After the completion of the education session, students will verbalize 3 health snack options.

Long term goal example:

In 6 months after the education session, the elementary school will note a decrease in COVID-19 positive cases by 10%.

Short term goal example

-After completion of presentation, residents will demonstrate proper handwashing for 2 minutes.

-After the completion of the presentation, resident will know and able to verbalize the importance of handwashing.

Long term goal example

-In 10 months after the completion of presentation, will be used to using the 10 locations or will remember where hand washing can be performed with the help of poster.

In this section you will:

1. Discuss your community project in detail.

2. How it will address the nursing diagnosis

3. What progress you have made so far in meeting your goals/objectives

Will be doing a PowerPoint presentation to both staff and resident on the Importance of hand washing and proper or idle way of hand washing process. Including signs at various hand washing location to help resident while performing hand washing at various times.

It will bring awareness to residents that do not know the importance of handwashing and what handwashing could prevent. It will also make both staff and residents cultivate the habit of handwashing.

Met with the Lead Nurse to discuss about how and when I will be doing the presentation, what location would be the best to do it at, what time of the day would be the best, type of signs that would create the awareness of the presentation.
